This page details how Microtransactions work in Dr. Mario World. In order to play as long as you want, you’ll need to purchase Diamonds with real-world money but in-game currency, in the form of coins, can be used to buy characters, items, and more.
Below is a breakdown of what you can buy (and what you need to buy) in Dr. Mario World.
The only thing you’d need to buy with real money is the ability to play stages but even that can be restored by just waiting (in real time).
Microtransactions, Hearts, and Infinite Play Explained[edit]
It costs 1 heart each time you attempt a level in stage mode (i.e the solo campaign in Dr. Mario World).
How to Get Hearts[edit]
- You get 1 heart every 30 minutes and can have a maximum of 5 hearts.
- You can receive hearts from friend. But a player can only give 1 heart to 1 friend per day.
- You get 1 heart for clearing a stage. Note: You do not get hearts for clearing a stage that has already been cleared.
- Misc prizes via question mark blocks.
How to Get Infinite Hearts[edit]
- When you first play Dr. Mario World you are given 60 minutes of infinite hearts, allowing you to play for an hour before using any hearts.
- After this initial 60 minutes, you will have to pay 30 Diamonds for infinite hearts.

Diamonds and Purchasing Options[edit]
You can use Diamonds to buy the following…
- Hearts
- Characters i.e Doctors and Assistants.
- Capsules: to continue playing a level if you run out of capsules early.
- Skill Up: an item that lets you start a level with the skill meter full.
- More time in challenge levels
- Hearts let you play stages in the main campaign.
- Characters have different stats that affect the game when playing alone and when playing competitively online.
How Much Do Diamonds Costs?[edit]
Below is a screenshot followed by a table breaking down the price of Diamonds.
Note: Diamonds can only be bought in bundles. Below is a table with all the purchasing options. Diamonds can only be bought with real money. They cannot be earned.

There are also rotating “special packs” that include diamonds, and items such as capsule boosts, skill fillers, hammers, etc. And a rotating diamond deal that’s not reflected in the above chart.
How to Buy and Unlock Characters[edit]
Characters can be bought from the staffing menu with Diamonds (which cost real money) or bought with in-game currency (coins you earn by playing).
Characters cannot be purchased until after level 20.
- Characters cannot be individually purchased. They appear at random. It could be a Doctor or an Assistant Character.
- Characters cost 4,000 coins or 40 Diamonds.
- If you get a character you already have, that character’s skill level will increase instead
- Once you reach the maximum skill level fr a specific character, that character will be removed from the pool of available characters.
Characters List: Doctors and Assistants[edit]
Doctors are the main character you play as and doctors can have up to 2 Assistants. Each character Doctor has their own stats/skill moves and each assistant has its own bonuses. All characters have a skill level.
